Form submissions are stored through Fluent Forms on our WordPress website. Terry J. Hodgkinson receives an email notification containing a link to view the submission.
Terry J. Hodgkinson and Dr. Bronwen Gates review these submissions for the purposes described above. Providers supporting our website, hosting and email systems may process information as part of operating those services.
Other disclosures may occur with your consent or where permitted or required by law.
How long we keep information
Enquiries that do not become client records: We retain enquiry and consultation-request forms for 12 months after our last contact, provided no clinical assessment took place and no other recordkeeping obligation applies.
Client records: We retain client intake forms and related client records for 15 years after the last entry in the client record.
For medical records involving children, we retain records for the longer of this period or 10 years after the client’s 18th birthday.
Records may be retained longer where necessary to meet legal or professional obligations, address a complaint or legal proceeding, or complete an outstanding access request.
At the end of the applicable retention period, records are scheduled for secure deletion or destruction.
